Any person (either acting on their own behalf or on the behalf of another person) accessing, browsing or otherwise using the K.I.D.S. IQ Project (“K.I.D.S.”, “K.I.D.S. 2. Personal Health Information
Coded Personal Health Information is stripped of direct identifiers such as names and addresses but, in each case, is assigned a numeric ID or code; this numeric ID or code is still linked to identifiable information, however, such linkages are not known to intended recipients of coded Personal Health Information. K.I.D.S. IQ may utilize third party service providers, such as email, website and database hosting companies, acting on behalf of K.I.D.S. IQ, to collect, store or share Patient Health Information that is not coded. If your consent is provided, Patient Health Information that is not coded may also be shared with individuals, families, researchers or others (as specified at the time of such consent). Once Patient Health Information is coded, K.I.D.S. IQ may share/publish such coded Personal Health Information on its Site, in addition to sharing it with researchers, entities affiliated with K.I.D.S. IQ or acting on behalf of/with K.I.D.S. IQ and relevant third parties such as K.I.D.S. IQ program participants and grant providers. By submitting Personal Health Information through the Site, you authorize K.I.D.S. IQ to share this coded Personal Health Information for the purposes identified, such as determining qualifications for grants, promoting syndrome awareness, organizing fundraising events, responding to User inquiries, processing donations, assisting researchers or determining eligibility for K.I.D.S. IQ programs. Once coded Personal Health Information is shared with an entity or person as it relates to the aforementioned purposes, it may then in turn be reproduced in an original or derivative format and shared publicly, for example, through the publication of a scientific/academic research paper. If you do not want K.I.D.S. IQ to disclose your coded information to third parties as described above, you may opt-out by submitting a written request to K.I.D.S. IQ at K.I.D.S. IQ Project, P.O. 3. Legal and Regulatory
